The condition and its result do not necessarily refer to the same time. This allows English to describe relationships between past causes and present results or present conditions and past results.
If I had studied medicine, I would be a doctor now.

A past unreal condition can have a present result.
The if-clause normally uses the Past Perfect, while the main clause uses would + base verb. For example, If I had studied medicine, I would be a doctor now.

A present or general unreal condition can be connected to an unreal result in the past.
